Chronic Inflammatory Response Syndrome (CIRS)
CIRS is a complex, persistent, multi-system inflammatory illness described in susceptible individuals following exposure to biologically contaminated environments, particularly damp and water-damaged buildings.
Chronic Inflammatory Response Syndrome, commonly abbreviated as CIRS, is used to describe a severe pattern of chronic inflammation involving multiple systems of the body. It has been associated with exposure to biologically produced substances found in water-damaged buildings, including mould particles, fungal fragments, bacterial contaminants, Actinomycetes, endotoxins, microbial metabolites and other inflammatory components of contaminated indoor dust and air.
CIRS is different from a conventional mould allergy. A typical allergy may cause relatively immediate symptoms such as sneezing, itchy eyes, nasal congestion, coughing or wheezing. CIRS is described as a broader and more persistent inflammatory response that may affect neurological, respiratory, muscular, hormonal, digestive, immune and metabolic function. The symptoms may appear unrelated, fluctuate over time and resemble several other chronic illnesses, which can make diagnosis difficult.
CIRS Is Not About One Mould Species
Water-damaged buildings rarely contain only one mould genus or one type of contaminant. Occupants may be exposed to a complex indoor microbial ecosystem containing mould spores, hyphal fragments, fungal dust, β-glucans, allergens, microbial volatile organic compounds, bacteria, endotoxins, Actinomycetes and, where produced under suitable conditions, mycotoxins.
The concern is therefore not limited to so-called “toxic black mould” or to the most damaging fungal species. Any mould genus that becomes significantly overabundant indoors may contribute to an unhealthy environmental burden and may affect mould-sensitised, chronically ill or immunocompromised occupants.
Why Do Only Some People Develop CIRS?
Not every person exposed to a damp or mould-contaminated building develops CIRS. Current clinical models suggest that individual susceptibility may be influenced by genetics, immune regulation, exposure intensity, duration of exposure, previous illness, respiratory health and other personal factors. Particular HLA-DR and HLA-DQ genetic patterns have been proposed as possible susceptibility factors because they may influence how the immune system recognises and responds to certain environmental antigens and biotoxins.
In susceptible individuals, exposure may contribute to ongoing activation and dysregulation of the innate immune system. Instead of the inflammatory response settling once the immediate exposure has passed, inflammatory signalling may continue, producing persistent symptoms across several bodily systems. This is sometimes described as the immune system remaining in a prolonged state of activation rather than returning efficiently to its normal regulated condition.
CIRS May Affect Multiple Body Systems
People being medically investigated for suspected CIRS may report a wide and changing combination of symptoms, including:
- Persistent fatigue and reduced stamina.
- Brain fog, poor concentration, memory difficulties and difficulty finding words.
- Headaches, dizziness, disorientation or sensitivity to light and sound.
- Muscle cramps, weakness, morning stiffness, joint pain and migrating body aches.
- Chronic sinus congestion, coughing, chest tightness or unexplained shortness of breath.
- Sleep disruption, temperature instability, sweats or sudden chills.
- Excessive thirst, frequent urination or difficulty maintaining normal hydration.
- Abdominal discomfort, diarrhoea, altered appetite or a metallic taste.
- Mood changes, irritability, anxiety or symptoms that may be mistaken for depression.
These symptoms are not unique to CIRS and may occur in many other medical conditions. A symptom list alone cannot confirm that a person has CIRS or that a particular building caused their illness. Proper assessment requires a detailed medical history, investigation of alternative diagnoses, consideration of environmental exposure and interpretation by a healthcare practitioner experienced in chronic inflammatory and environmentally acquired illness.

International Research and Our Professional Experience
Over the past two decades, international research has increasingly recognised that prolonged exposure to damp and water-damaged buildings may contribute to a wide range of adverse health effects. Studies from the United States, Belgium, Finland, Spain, Australia and other countries, together with publications from the World Health Organization (WHO), have consistently demonstrated associations between damp indoor environments and increased respiratory disease, asthma, allergic conditions, chronic sinusitis, fatigue, neurological symptoms and persistent inflammatory responses. While research into CIRS continues to evolve, there is broad scientific agreement that moisture-damaged buildings can significantly compromise indoor environmental quality and adversely affect human health.
Modern research has also shown that occupants of water-damaged buildings are exposed to considerably more than airborne mould spores. Indoor environments affected by chronic moisture intrusion may contain a complex mixture of mould spores, microscopic hyphal fragments, fungal cell-wall components such as β-glucans, microbial volatile organic compounds (mVOCs), bacteria, endotoxins, Actinomycetes, contaminated settled dust and, where environmental conditions permit, mycotoxins. Collectively, these biological contaminants create what many researchers now describe as an unhealthy indoor microbial ecosystem, capable of placing continuous inflammatory stress on susceptible occupants.
The Focus Has Shifted From “One Dangerous Mould” To The Entire Indoor Microbial Environment
Current indoor environmental science increasingly recognises that health risks within water-damaged buildings cannot be explained by a single mould species or by mycotoxins alone. Instead, the combined effects of persistent moisture, microbial overabundance, fungal diversity, bacterial contamination, airborne particulates and reduced indoor air quality create an environment capable of affecting mould-sensitised individuals, people with chronic respiratory disease and immunocompromised occupants. Understanding the entire microbial ecology of a building is therefore considerably more valuable than simply identifying one mould genus.
